A change request is per person. If you would like to submit a change for your spouse/child(ren), you must submit the online form for each family member separately.    
 

You will need 

- SZVIDnr. (available in your MySZV account or on your valid/expired insurance card)  
- Active e-mail address to receive your e-mail confirmation.  
- Active phone number.  

Your confirmed doctor change request will go into effect as of April 1, 2021.
