Dr. Ruth Douglass will no longer attend to clients insured with a SZV medical insurance due to conflict of interest, effective January 15, 2019.

Only patients of Dr. Ruth Douglass can change their family doctors at the SZV starting now until February 28, 2019 using the online form.

It is no longer possible to make use of this procedure. Patients wishing to change doctor can make use of the March Doctor change option. Click here for more information.

ZV, 60+/62 +, OZR and PP insured patients: 

• IMPORTANT: One person per form. Each family member must fill in a separate/individual form with their change request. 
• Once you have submitted the form, SZV will inform you of the pick-up date of your new medical insurance card

FZOG insured patients: you do not have to inform SZV of your change of doctor.
