It is the duty of the insured whose designated country of care is Colombia or The Dominican Republic to inform the medical referral officer at the time of being informed of the prospective travel time if they are unable to make the indicated appointment date.
In the case of the designated country being Saint Martin or Curacao, it is the duty of the insured to inform the medical referral officer at the time of notification of the appointment if they cannot make the appointment.
The insured will be required to come in to SZV within 24hrs (or email in the signed waiver) and sign a waiver regarding the postponement of the obtained appointment. Any cancellations after the indicated periods above will result in the insured being responsible for all fees related to the late cancellation (ticket changes, hotel fees) and the new appointment will not be made until the fees has been paid. The date of the new appointment will be at the discretion of the physician to whom the insured is referred. If the insured refuses to sign the postponement of appointment waiver (or the waiver is not received within 5 working days), the appointment will be cancelled and a new referral will have to be submitted for evaluation if the insured requests a new appointment at a later date.
